How remote monitoring works

With MediConnect®, we offer you intelligent and structured telemonitoring for your heart failure patients - G-BA approved and guideline-compliant. 

Data is transmitted daily from the patients to the Telemedicine Center (TMC), where it is continuously analyzed by MediConnect® E-Health. Data from internal devices, such as ICDs or biventricular systems, are transmitted and analyzed. For example, the heart rhythm at rest and during physical activity or data on the functionality of the internal device. Patients without an implanted device can determine the necessary data themselves using external devices and transmit their current body weight, measured blood pressure, ECG and their own assessment of their state of health electronically to the TMC.

German G-BA decision as a basis

Since March 31, 2021, seamless telemedical care for patients with advanced heart failure has been part of the outpatient services offered by statutory health insurance companies. 

The care approach is based on cooperation between TMC and primary care physicians (PBA). Patients' vital signs are no longer only recorded occasionally but continuously. Deviations, e.g., in heart function, can be detected quickly, and therapy can be adjusted accordingly.

Inclusion criteria for patients

  • Diagnosed heart failure
  • NYHA classification NYHA II or NYHA III
  • Ejection fraction less than 40%
  • ICD, CRT-P or CRT-D
  • Inpatient admission due to cardiac decompensation in the last 12 months

TMC Tasks

The TMC manages data management, including the patient's technical equipment. The medically trained TMC staff register abnormal findings and inform the physician promptly, at the latest on the working day following the data transfer. The TMC would only provide care at off-peak times, e.g., outside practice opening hours or in special cases where intensive monitoring of individual symptoms appears necessary.

 

Physician Tasks

The direct treatment decision is always in the hands of the primary treating physicians (PBA). The PBA must confirm the data provided by TMC within 48 hours via the MediConnect® web viewer. The PBA then informs the TMC of the treatment measures initiated and discusses them with the patient. If there are no abnormalities, the case is closed.

 

Telemonitoring with external devices

As part of telemonitoring, data such as ECG, blood pressure, weight, SPO2, or information collected for self-assessment of the patient's state of health are transmitted securely, in an encrypted form, and are stored and displayed in MediConnect® E-Health.

Fleischhacker supplies the necessary external sensors to the telemedicine center, which makes them available to the patient. The standard equipment includes a tablet with a patient app, a scale, and a combined ECG and blood pressure monitor.

 

Digital Scale

This digital scale displays your body weight and provides 15 measurements from BMI to heart rate, muscle mass, etc., thanks to state-of-the-art bioimpedance analysis. It is your perfect companion for telemedical monitoring as part of the ACTIMI device package.

 

Blood pressure monitor and ECG device

This wireless blood pressure monitor with ECG combines a measuring cuff and blood pressure monitor without unwanted parts such as tubes or cables. It measures systolic and diastolic blood pressure and pulse rate. It is part of the ACTIMI external device package.

 

Tablet with questionnaire

Blood pressure, ECG and weight are measured using the devices supplied in the package and transmitted to the app on the tablet. In addition, the patient answers some important questions about their general condition, which is sent to the TMC together with the values.

 

Telemonitoring with internal devices

Every year, more than 150,000 implantable cardioverter defibrillators (ICD), and implantable loop recorders (ILR) alone  are implanted in Germany in pacemakers.. Due to demographic developments, the trend is rising. The systems must be checked every 3 to 6 months by the guidelines. It is estimated that there are approximately 1.2 to 1.6 million patients with implants in Germany who require regular follow-up care.

This is an ideal field of application for the MediConnect® E-Heath module. MediConnect® E-Health supplies itself daily with all the data provided by the manufacturer platforms and displays it accordingly.

An additional optimization of this telemonitoring procedure is the optional interaction with the device implantation modules and the functional diagnostic module Device Aftercare.

Statistical evaluations according to German GBA decision

At the touch of a button, MediConnect® provides you with all the parameters required for the annual statistics following the GBA resolution, which are transmitted electronically to the National Association of Statutory Health Insurance Physicians. These include - as a median and mean value - the age of the patients, the proportion of notifications to the PBA per patient, or the number of inpatient admissions due to cardiac decompensation in patients.
